WE WELCOME NEW PATIENTS
Your comprehensive examination in our office include teeth, gums, and surrounding tissue as well as an oral cancer exam and a series of X-rays. We will evaluate your general dental health as well as look for cavities and possible gum disease.
WHAT'S REQUIRED
Filling out our personal questionnaire and medical information forms will allow us to serve you better. Please print off New Patient forms from our website, fill them out and bring them to your appointment with you. If this is not possible, please arrive 10 minutes early to fill them out before your appointment begins.
FINANCIAL OPTIONS
We accept Cash, Cheque, Debit Card, Visa, MasterCard & American Express. If you have special needs or circumstances, please discuss this with the administrative staff PRIOR to your appointment.
INSURANCE OPTIONS
We will be happy to help you submit your claim electronically from our office providing you with a copy of the insurance company response. As a courtesy to you, benefits from the insurance company can be sent to us directly if your insurance company will allow this. However, should there be a balance, you must pay this at the time of your appointment. If the insurance company does not respond with the percentage paid and balance owing, it is your responsibility to pay for this appointment and be reimbursed from your insurance company. It is not the responsibility of this office to follow up on claims outstanding without payment but will assist you in any way we can.
Patients are reminded that their insurance policy is an agreement between them and their insurance company and not between the insurance company and this office. We are committed to helping you understand your benefits, should you provide us with your policy booklet.
